Justice‑Involved Case Manager (Reentry & Prison Coordination)

Position Summary
We are seeking motivated, mission-driven Case Managers to support justice‑involved individuals preparing for release and successful reentry into the community .
This role focuses on serving individuals connected to Arizona correctional facilities, probation/parole systems, and reentry programs , ensuring continuity of care, compliance with court/program requirements, and long-term stability.
The ideal candidate understands that reentry is not just about release-it's about structure, accountability, and support across multiple systems.
